New Patient Appointment Scheduling
(You will need to do the following)
- To make an appointment please call our main number 512.450.1300.
- Office Hours are Mon-Fri. 8:00 am to 5:00 pm.
- Have your insurance card available so that we can help you obtain the optimum benefit. We are contracted with most managed care plans.
- We will make every effort to schedule your appointment for your convenience. If your injury requires same or next day treatment, we will do our best to work you in.
- To save time, you may print and complete the required new patient forms on-line prior to your appointment. If not, Please arrive 20 minutes prior to your scheduled appointment time to complete the required forms in our office.
- If your insurance requires a referral from your primary care physician, please contact your doctor to request the referral. Many offices require 24-48 hours to process the referral and forward it to us.
- Please have your insurance card and driver’s license available.
- If you have x-rays, MRIs or other imaging studies related to your injury, please bring them to your appointment. This will allow us to avoid duplicating the service and related cost.
- We will collect co-payments, as required by your insurance plan, at the time of your appointment. We accept cash, checks and all major credit cards.
We are conveniently located in the Central Park Medical Office Building on the Northeast corner of 38th and Lamar by the Heart Hospital of Austin and Central Market.
Parking is available in the Central Park Garage located on the behind / east side of the building at a cost of $1.50 per hour. Valet parking is also available at the front entrance at a cost of $1.50 per hour.
Handicap access, automatic doors and valet assistance is provided at the front entrance.
